Case Description
This webinar focuses on advanced guidewire techniques for crossing complex coronary lesions during percutaneous coronary intervention (PCI). It explores strategies for managing challenging lesions, including severe stenosis, tortuous vessels, calcified lesions, and chronic total occlusions. The...
Case Summary
- Complex percutaneous coronary intervention frequently involves challenging anatomical subsets, including severe calcification, marked vessel tortuosity, bifurcation disease, and chronic total occlusions, which collectively account for approximately twenty percent of coronary crossing challenges. Successful guide wire traversal represents the primary determinant of procedural success, and systematic wire selection reduces procedural complications by up to forty percent. Pre-procedural evaluation requires thorough angiographic assessment of proximal cap morphology, occlusion length, vessel trajectory, collateral pathways, and distal runoff quality, appropriately stratified using clinical scoring systems such as the Japanese Chronic Total Occlusion score. Complementary intravascular ultrasound and optical coherence tomography provide vital diagnostic insight into plaque composition and microchannel architecture.
